bof do not know much about Japanese resturuants but local restuarants in old city area are worth trying.

I know 3 Japanese restaurants in Phuket which are:

The Otawa Japanese Restaurant

You also have the Todai Japanese Restaurant

However, as said above, I wouldn't go to Phuket to eat Japanese food ;-)
